CARE PLAN - WHAT IS IT?
Nursing diagnoses and Care Planning are an essential part of the daily care of every patient.
On admission to the hospital, after patient assessment and history and physical has been done by the nurse, the doctor examines and diagnoses the patient's condition.
Patient problems revolve around the diagnosis made.The nursing kardex is used by nurses to provide information and the daily plan of care.
Some examples: Sally is being admitted to the hospital for surgery, she has never been in a hospital before. So anxiety related to hospitalization and patient education would need to be addressed as part of the care plan. Potential for Paralytic Ileus and postop pneumonia would also be some of her problems.
